Fall Sports Physicals Signups

Dr. Young our school physician will be completing physicals for fall sports on Monday, June 3rd and Tuesday, June 4th at the High School/Middle School Nurse's office. Physicals will starting around 8:00 am. To give permission for your student to receive a physical, please follow the link to complete the jotform. This will also allow us to send email reminders leading up to the day of physicals. Additionally, the health staff will be available at these times to help any parent/guardian with the rest of the BigTeams physical paperwork as we no longer use paper forms.
